FILE A CLAIM

Pursuant to the Court's Order filed on October 14, 2022, Class Members who wish to participate in the Settlement shall complete and submit the Proof of Claim and Release form ("Proof of Claim" or "Claim Form") in accordance with the instructions contained therein. To obtain a copy of the Claim Form, click here. The deadline to submit a timely Claim Form is February 2, 2023.

If you fail to execute and submit a timely, properly completed and addressed Claim Form, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.
